Compass Pathways Accelerates Commercial Preparations as COMP360 Approaches Potential FDA Approval

The Nasdaq-listed psychedelic medicine developer is expanding its workforce, preparing a US sales operation and engaging healthcare providers ahead of a potential regulatory decision in 2027.

Compass Pathways (Nasdaq: CMPS) is expanding preparations for the potential US commercial launch of COMP360, its investigational synthetic psilocybin treatment for treatment-resistant depression, ahead of an anticipated regulatory decision in 2027.

Chief Executive Officer Kabir Nath has confirmed that Compass has added approximately 150 employees, expanded its marketing capabilities and established new offices in Jersey City as it prepares for a possible US Food and Drug Administration (FDA) decision in the first half of next year.

In an interview with Business Insider published on 25 October, Nath discussed the company’s preparations to introduce COMP360 into the American healthcare system, including the clinical infrastructure and commercial operations needed to support treatment.

The investment comes ahead of regulatory approval, with Compass committing resources to a potential launch while its lead drug candidate remains under development.

Clinical Results Support Commercial Planning

COMP360 is a proprietary synthetic psilocybin formulation being developed for adults with treatment-resistant depression, generally defined as depression that has not responded adequately to at least two antidepressant treatments.

The programme has received FDA Breakthrough Therapy designation, which is intended to facilitate the development and review of treatments that may offer substantial improvements over existing options for serious conditions.

According to clinical trial results reported by Compass in September, just under half of participants receiving the investigational treatment experienced a reduction of at least 50% in depressive symptoms within six weeks.

The findings have supported the company’s continued development programme and commercial planning, although the full clinical evidence, including safety, durability of response and the overall benefit-risk profile, remains subject to regulatory assessment.

Compass is preparing its remaining regulatory documentation and is targeting a potential FDA decision in the first half of 2027.

Preparing Healthcare Providers for COMP360

A central part of Compass’s commercial preparations involves establishing the clinical infrastructure needed to administer COMP360.

Unlike conventional antidepressants dispensed through community pharmacies, COMP360 is administered in a supervised clinical setting.

Under the company’s clinical trial protocols, patients receive an oral dose of synthetic psilocybin following preparation with a trained healthcare professional. The dosing session takes place in a controlled environment, with professional supervision throughout the psychedelic experience.

Sessions typically last approximately six hours, requiring treatment centres to accommodate extended patient appointments and allocate appropriately trained staff.

Nath has explained that the company’s treatment model does not necessarily involve formal psychotherapy during the dosing session. Instead, healthcare professionals provide supervision, psychological support when required and ongoing safety monitoring.

This approach may influence the cost and operational requirements of delivering COMP360 compared with psychedelic-assisted therapies that incorporate more extensive psychotherapy.

Staff training, treatment-room availability, patient screening and reimbursement arrangements will all need to be addressed before the treatment can be offered widely.

Compass has begun engaging psychiatrists and other healthcare providers about incorporating COMP360 into existing clinical services, alongside preparations for a dedicated US commercial sales operation.

Competition and Commercial Challenges

Compass is among the most advanced publicly listed developers of psychedelic-based psychiatric medicines targeting the US pharmaceutical market.

Its progress has attracted attention from investors and other drug developers, particularly following the FDA’s decision not to approve Lykos Therapeutics’ MDMA-assisted therapy application in 2024.

The agency requested additional clinical evidence from Lykos, highlighting the regulatory challenges facing developers of psychedelic medicines, including questions around trial design, treatment protocols and the assessment of clinical benefit.

For Compass, COMP360 remains its principal development asset, making the outcome of its regulatory programme particularly important to the company’s commercial prospects.

Competition is also developing among companies pursuing psychedelic treatments with different pharmacological profiles and administration requirements.

AtaiBeckley, for example, is developing an intranasal DMT-based treatment, while several other developers are investigating shorter-duration psychedelic compounds that may require less time in a supervised clinical environment.

Treatment duration could influence how providers evaluate these medicines, particularly where staffing costs, appointment capacity and reimbursement affect the financial viability of treatment services.

Even with regulatory approval, Compass would need to establish a commercially sustainable model for delivering COMP360 across a healthcare system that has limited existing infrastructure for psychedelic treatment.

Implications for Australia and International Markets

Compass’s development programme is also relevant to Australia’s emerging psychedelic medicine sector, where regulated prescribing pathways have been available since July 2023.

Under the Therapeutic Goods Administration’s Authorised Prescriber scheme, specifically authorised psychiatrists can prescribe certain unapproved MDMA products for post-traumatic stress disorder and psilocybin products for treatment-resistant depression, subject to regulatory conditions.

The Australian framework differs from the pharmaceutical approval pathway being pursued by Compass in the United States.

While Australian psychiatrists can access eligible unapproved psychedelic medicines through the Authorised Prescriber scheme, Compass is seeking formal FDA approval for a proprietary pharmaceutical product supported by its clinical development programme.

US approval could encourage further investment in psychedelic drug development and provide useful commercial evidence for healthcare organisations considering how these treatments might be delivered.

However, FDA approval would not automatically authorise COMP360 for routine prescribing in Australia, nor would it address the reimbursement and affordability challenges facing the local sector.

Looking Ahead to 2027

Compass’s recruitment, marketing investment and engagement with healthcare providers suggest that management intends to have much of its commercial infrastructure in place before receiving a final regulatory decision.

The approach carries financial risk, particularly if the FDA requires additional evidence or the anticipated approval timeline changes.

Even if COMP360 secures approval, the company will face further challenges in establishing treatment access, securing reimbursement arrangements and encouraging adoption among psychiatrists and healthcare providers.

The next phase of Compass’s development will therefore involve both regulatory progress and continued investment in the practical requirements of delivering its treatment.

With a potential FDA decision anticipated in the first half of 2027, Compass is approaching an important period for its clinical programme and commercial ambitions.
