Incannex Healthcare Advances Clinical Pipeline Across Sleep Apnoea, Arthritis and Anxiety

The Australian-founded biotechnology company is progressing three investigational treatments, including a synthetic psilocybin therapy for generalised anxiety disorder, as its lead sleep apnoea programme moves closer to late-stage clinical development.

Nasdaq-listed biotechnology company Incannex Healthcare (NASDAQ: IXHL) is advancing three clinical programmes targeting obstructive sleep apnoea, rheumatoid arthritis and generalised anxiety disorder, with its lead drug candidate progressing towards Phase 3 development.

The Australian-founded company has built its pipeline around medicines that combine existing pharmacological compounds, alongside a separate programme investigating synthetic psilocybin administered with psychological support.

Its development strategy reflects an interest in treatments that act through multiple mechanisms, particularly for conditions where existing therapies may be ineffective, poorly tolerated or difficult for patients to maintain.

Incannex’s most advanced programme is IHL-42X, an oral combination treatment for obstructive sleep apnoea. The company is also developing IHL-675A for rheumatoid arthritis and PSX-001, a synthetic psilocybin candidate for generalised anxiety disorder.

IHL-42X: Developing an oral treatment for sleep apnoea

Obstructive sleep apnoea (OSA) is a common sleep disorder characterised by repeated interruptions to breathing during sleep. It can cause daytime fatigue, impaired concentration and increased cardiovascular risk.

Continuous positive airway pressure (CPAP) remains an established treatment, although some patients struggle with the equipment or find it difficult to maintain consistent use.

Incannex is developing IHL-42X as a potential oral alternative. The treatment combines dronabinol, a synthetic form of THC, with acetazolamide, a medicine already used for several conditions, including glaucoma and altitude sickness.

The two compounds act on different physiological processes involved in breathing regulation, with the combination intended to reduce the frequency of breathing interruptions during sleep.

In August 2026, Incannex announced that the first patient had been dosed in its DReAMzz Phase 2 dose-optimisation study, following positive results from its earlier RePOSA trial.

The RePOSA study reported statistically significant reductions in the apnoea-hypopnoea index (AHI), a measure of sleep apnoea severity, compared with placebo. Individual reductions of up to 83% were observed, although this figure represents outcomes in particular participants rather than the average treatment effect.

The DReAMzz study is designed to refine dosing and provide additional evidence to support a planned Phase 3 clinical programme.

IHL-42X has also received Fast Track designation from the US Food and Drug Administration (FDA). In June 2026, Incannex secured a US patent covering the treatment’s composition and associated methods, with a baseline expiry in July 2040.

IHL-675A: Investigating a combination treatment for rheumatoid arthritis

Incannex’s second programme, IHL-675A, combines synthetic cannabidiol (CBD) with hydroxychloroquine sulfate, an established medicine used in the treatment of rheumatoid arthritis and other autoimmune conditions.

Rheumatoid arthritis is a chronic inflammatory disease in which the immune system attacks the joints, potentially causing pain, swelling and progressive damage.

Although several effective treatments are available, some patients experience inadequate responses or adverse effects that limit their options.

Incannex is investigating whether combining CBD and hydroxychloroquine can provide additional anti-inflammatory benefits through their different mechanisms of action.

IHL-675A has completed Phase 1 safety and pharmacokinetic testing and is in Phase 2 clinical development.

The programme faces an established and increasingly sophisticated treatment market, including conventional disease-modifying medicines and newer biologic therapies. Any clinical advantage will need to be demonstrated against that existing standard of care.

PSX-001: Synthetic psilocybin for generalised anxiety disorder

Incannex’s third programme is particularly relevant to the emerging field of psychedelic medicine.

PSX-001 is an investigational oral synthetic psilocybin treatment being developed for adults with generalised anxiety disorder (GAD), a condition characterised by persistent and excessive anxiety that can substantially interfere with daily life.

The treatment is being investigated alongside structured psychological support, with the aim of assessing whether psilocybin-assisted treatment can provide meaningful and sustained improvements in anxiety symptoms.

Incannex completed dosing in its initial Phase 2 PsiGAD1 study in January 2024 and subsequently reported positive results.

The company has since progressed its clinical and regulatory planning for PsiGAD2, a Phase 2b dose-comparison study designed to investigate different doses of psilocybin and further assess safety and efficacy.

The programme has received FDA clearance for an Investigational New Drug application, allowing clinical investigation under the relevant US regulatory framework. Incannex has also engaged with the UK’s Medicines and Healthcare products Regulatory Agency regarding its development plans.

Psilocybin-assisted therapy has generated considerable research interest across psychiatric conditions, including depression, post-traumatic stress disorder and anxiety. However, questions remain around optimal dosing, durability of treatment effects, patient selection and how psychological support should be incorporated into clinical delivery.

For Incannex, further controlled clinical data will be necessary to establish whether PSX-001 offers a meaningful treatment option for people with generalised anxiety disorder.

A pipeline built around different treatment approaches

While Incannex describes its broader strategy in terms of combination therapies, its three programmes differ considerably in their clinical approach.

IHL-42X and IHL-675A are fixed-dose pharmaceutical combinations intended to influence multiple biological pathways. PSX-001 involves a psychedelic compound administered within a structured treatment setting, where psychological support forms part of the clinical model.

These differences carry practical implications for development, regulatory assessment and eventual delivery.

An oral treatment for sleep apnoea, for example, would be evaluated against established sleep medicine outcomes and existing treatment options. Psilocybin-assisted therapy presents additional considerations around treatment supervision, psychological safety and the resources required to deliver care.

The company’s portfolio also spans three distinct therapeutic markets, each with different clinical endpoints, competitors and commercial considerations.

Clinical progress and the road ahead

In its September 2026 corporate update, Incannex reported approximately US$70 million in cash and cash equivalents as at 31 August, with no debt.

The company also reported receiving more than A$11.2 million through Australia’s Research and Development Tax Incentive programme during 2026, providing additional funding for its clinical development activities.

Its immediate clinical focus remains IHL-42X, with the DReAMzz study intended to inform the dosing strategy and design of a subsequent Phase 3 programme.

Meanwhile, the rheumatoid arthritis and psilocybin programmes provide exposure to different areas of pharmaceutical research, although both require further evidence before their potential clinical and commercial value can be properly assessed.

For Incannex, the coming stages of development will centre on generating that evidence, particularly as IHL-42X approaches later-stage trials and PSX-001 continues through its clinical and regulatory programme.

The company’s progress across three indications provides several potential development pathways, but each candidate will ultimately be assessed on its own clinical results.
